Your total compensation is comprised of both your salary and the array of benefits offered to you as a benefits-eligible employee while at the city.

To ensure you have successfully completed all the required paperwork to enroll in benefits, we are providing you with a checklist. This checklist is designed to help you make your initial benefit enrollment selections in a timely manner. Your benefits will begin the first day of the month following your date of hire, so it is imperative that all paperwork is completed and returned to Human Resources as quickly as possible. We do recognize that making benefit selection decisions is important, but we want to make sure that everything has been processed correctly, so that you don’t experience any headaches regarding your benefits. Please make your selections carefully. You may not change your medical, dental, vision or life insurance elections until the next Open Enrollment period unless you experience a qualifying Life Status Change Event.

Checklist
    I have completed the Annual Benefit Selection Form.
    I have gathered all required spousal and dependent supporting documentation.
    I have completed the Medical Mutual of Ohio Medical Enrollment Form (even if I am waiving medical insurance, I have completed the waive section)
    I have completed the Guardian Enrollment Form for dental, vision and life insurance (even if I am waiving dental and vision coverage, I have completed the waive section)
    If I have chosen the HSA Plan, I have completed the HSA Payroll Deduction Form
    If my previous employer is or I have previous employment with a municipality or state/public agency, I have completed the Vacation Credit Form and contacted the employer to send information to Michael L. Brown, Director of Human Resources
